Job Description:
As a Palliative Care Physiotherapist with Spectrum Health Care, you will play a vital role in enhancing the quality of life for patients and their families. Your expertise will focus on providing holistic proactive, seamless care and support to clients, families, and caregivers, promoting dignity, security, and comfort throughout the end-of-life journey.
Key Responsibilities:
• Conduct assessments and develop treatment plans tailored to individual patient needs and goals of care.
• Collaborate with an interdisciplinary team to create personalized care plans based on patient-specific requirements.
• Participate in clinical huddles and case conferences with healthcare professionals to ensure comprehensive care coordination.
• Engage in skilled communication with patients, families, and caregivers regarding mobility, function, and end-of-life care goals.
• Educate and mentor patients, family, and caregivers on strategies to maintain safe mobility and function in home settings.
• Maintain accurate and timely documentation of patient assessments, treatment plans, and outcomes, adhering to ethical and legal standards.
Requirements:
• Master's degree in Occupational Therapy or equivalent experience.
• Registration with the College of Physiotherapists of Ontario in good standing.
• 3-5 years of experience working as a Physiotherapist, preferably in palliative care settings.
• Successful completion of any of the following is considered an asset: Learning Essentials Approaches to Palliative and End of Life Care (LEAP); Fundamentals of Palliative Care; The Comprehensive Advanced Palliative Care Education (CAPCE).
• Valid Ontario driver's license and reliable vehicle for travel to client locations.
• Ability to work autonomously and collaboratively as part of an integrated healthcare team.
• Expertise in managing complex physical function and mobility issues in palliative care is an asset.
• Proficiency in conducting assessments, interpreting findings, and formulating evidence-based treatment plans.
• Familiarity with assistive devices, adaptive equipment, and environmental modifications.
• Excellent communication and interpersonal skills.
• Strong clinical skills and passion for improving patient quality of life.
• Effective problem-solving and critical thinking abilities.
• Proficiency in computer software programs (e.g., Microsoft Office) and virtual communication platforms (e.g., Microsoft Teams, Zoom, Go To Meeting).
