Social Services Professional

We are seeking a highly skilled and compassionate Social Worker to join our team at Fraser Health Authority. As a Social Worker, you will play a vital role in providing psychosocial assessments and therapeutic clinical counseling services to clients in psychiatry.
Key Responsibilities:- Conduct thorough psychosocial assessments of clients through focused interviews and interpretation of data within the context of the client system.
- Develop and implement care plans that address the unique needs of each client, including referrals to community resources and services.
- Provide ongoing monitoring and evaluation of client progress, revising care plans as necessary.
- Collaborate with interdisciplinary teams to ensure comprehensive care and support for clients.
- Act as a liaison between psychiatric units and community agencies, facilitating referrals and resource linkage.
- Provide education and consultation to clients and healthcare professionals on issues related to social work practice in psychiatry.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Bachelor's Degree in Social Work from an accredited institution.
- Current full registration with the BC College of Social Workers.
- Valid BC Driver's license and access to a personal vehicle for business purposes.
- Excellent communication and interpersonal skills, with the ability to work effectively in a multicultural environment.
- Ability to prioritize workload and manage multiple tasks and responsibilities.
