Job Summary

We are seeking a highly skilled and compassionate Family Practitioner to join our team at the Community Transitional Care Program. As a key member of our healthcare team, you will provide high-quality care to clients with complex psychiatric needs, working collaboratively with other healthcare professionals to ensure seamless transitions and successful reintegration into the community.

Key Responsibilities

  • Provide primary care services to clients with mental health and substance use disorders
  • Collaborate with multidisciplinary teams to develop and implement individualized care plans
  • Conduct comprehensive assessments and provide ongoing care to clients with complex needs
  • Work closely with community partners to ensure smooth transitions and reintegration into the community
  • Participate in quality improvement initiatives and contribute to the development of best practices in forensic mental health care

Requirements

  • Eligibility for provisional or full licensure with the College of Physicians and Surgeons of British Columbia
  • Certification with the College of Family Physicians or equivalent
  • Experience working with patients with complex psychiatric needs and substance use disorders
  • Strong communication and interpersonal skills
  • Ability to work collaboratively as part of a multidisciplinary team
