Resource Consultant

We are seeking a highly skilled and compassionate Resource Consultant to join our team at Alzheimer Society Southwest Partners. As a key member of our community clinical services team, you will play a vital role in supporting individuals living with dementia and their care partners.
Key Responsibilities:- Provide education, coaching, and role modeling for Emergency Department team members on behavioral strategies to mitigate the risk or necessity for restraints.
- Assist existing Memory Clinic and Geriatric clinic teams in the community to perform cognitive testing as needed.
- Liaise with other community agencies to facilitate referrals and coordinate client services as required.
- Participate in coordinated care planning to ensure smooth transition out of the Emergency Department back into the community as needed.
- Represent the Alzheimer Society professionally in the community, treating all internal and external clients with respect and dignity.
- University degree in social sciences or a related field, or comparable education.
- Knowledge and understanding of community resources and services.
- Demonstrated experience working with persons living with dementia.
- Ability to communicate fluently in French or other language(s) is an asset.
- Experience working in an interprofessional team is an asset.
- Case management experience is an asset.
- Membership with a Professional College is preferred.
- Proficiency with Microsoft Office Suite applications.
- Valid driver's license and access to a vehicle.
