Resource Consultant

We are seeking a highly skilled and compassionate Resource Consultant to join our team at the Alzheimer Society Southwest Partners. As a key member of our team, you will play a vital role in supporting individuals living with dementia and their care partners.
Job Summary:The Resource Consultant will provide support and guidance to individuals living with dementia and their care partners, connecting them with community resources and services. This role will also involve educating and modeling behavioral strategies for healthcare team members to mitigate the risk of restraints in the Emergency Department.
Key Responsibilities:- Provide support and guidance to individuals living with dementia and their care partners, connecting them with community resources and services.
- Educate and model behavioral strategies for healthcare team members to mitigate the risk of restraints in the Emergency Department.
- Assist existing Memory Clinic and Geriatric clinic teams in the community to perform cognitive testing as needed.
- Provide families with relevant information for other community agencies and programs when appropriate.
- Liaise with other community agencies to facilitate referrals and coordinate client services as required.
- Participate in coordinated care planning to ensure smooth transition out of the Emergency Department back into the community as needed.
- University degree in social sciences or a related field, or comparable education.
- Knowledge and understanding of community resources and services.
- Demonstrated experience working with persons living with dementia.
- Ability to communicate fluently in French or other language(s) is an asset.
- Experience working in an interprofessional team is an asset.
- Case management experience is an asset.
- Membership with a Professional College is preferred.
- Proficiency with Microsoft Office Suite applications.
- A valid driver's license and access to a vehicle are required.
