About the Role

We are seeking a highly skilled and compassionate Dementia Resource Coordinator to join our team at the Alzheimer Society Southwest Partners. As a key member of our community support team, you will play a vital role in providing support and resources to individuals living with dementia and their care partners.

Key Responsibilities
  • Provide support and guidance to individuals living with dementia and their care partners, connecting them with community resources and services.
  • Collaborate with healthcare professionals and community agencies to ensure seamless care coordination and transition planning.
  • Develop and implement behavioral strategies to mitigate the risk of restraints in the Emergency Department, utilizing Montessori activities and GPA techniques.
  • Assist with cognitive testing and assessments as needed, working closely with memory and geriatric clinics in the community.
  • Provide education and coaching to Emergency Department staff on behavioral strategies and dementia care best practices.
  • Liaise with primary care providers to support stabilization in the community for individuals living with dementia.
  • Collect and analyze statistical data to inform program development and evaluation.
  • Represent the Alzheimer Society professionally in the community, upholding our values of equity, inclusion, and social justice.
Requirements
  • University degree in social sciences or a related field, or comparable education and experience.
  • Knowledge and understanding of community resources and services, with a focus on dementia care and support.
  • Experience working with individuals living with dementia and their care partners, with a strong understanding of behavioral strategies and GPA techniques.
  • Excellent communication and interpersonal skills, with the ability to work effectively in an interprofessional team.
  • Proficiency in Microsoft Office Suite applications and ability to learn new software systems.
  • Valid driver's license and access to a vehicle, with occasional travel required.
