About the Role

We are seeking a highly skilled and compassionate Dementia Resource Coordinator to join our team at the Alzheimer Society Southwest Partners. As a key member of our community clinical services team, you will play a vital role in supporting individuals living with dementia and their care partners.

Key Responsibilities
  • Provide support and guidance to individuals living with dementia and their care partners, connecting them to programs and services within the Alzheimer Society and other community resources.
  • Collaborate with healthcare professionals and community partners to ensure seamless transitions and coordinated care planning.
  • Develop and implement behavioral strategies to mitigate the risk of restraints in the Emergency Department, utilizing Montessori activities and GPA techniques.
  • Assist with cognitive testing and assessments as needed, working closely with memory and geriatric clinics in the community.
  • Represent the Alzheimer Society professionally in the community, upholding our values of equity, social justice, and respect for all individuals.
Requirements
  • University degree in social sciences or a related field, or comparable education and experience.
  • Knowledge and understanding of community resources and services, with a focus on dementia support.
  • Experience working with individuals living with dementia, with a strong understanding of Alzheimer's disease and related dementias.
  • Excellent communication and interpersonal skills, with the ability to work effectively in an interprofessional team.
  • Proficiency in Microsoft Office Suite applications and ability to maintain accurate records.
