Job Title: Intensive Case Management

We are seeking a highly skilled and compassionate professional to join our team as an Intensive Case Manager. In this role, you will work closely with clients who are struggling with substance use and mental health issues, providing them with the support and guidance they need to achieve recovery and stability.

Qualifications:
  • Bachelor's degree in a health-related field from an accredited institution;
  • Post-basic education in mental illness and/or substance use;
  • At least two years of experience working with clients with substance use and mental health issues in a community setting;
  • Strong knowledge of harm reduction philosophy and practice, as well as HIV, HCV, and related issues;
  • Experience working with individuals who have severe substance use problems and do not readily engage in healthcare services;
  • Understanding and application of substance misuse assessments, treatment methodologies, intervention strategies, the change process, and community dynamics;
  • Knowledge of applicable legislation, such as Mental Health and Child Protection legislation;
  • Knowledge of available programs and services, their interrelationships, and their function in delivering services in the community;
Skills and Abilities:
  • Demonstrated competence in the treatment of addiction and mental illness congruent with Island Health's framework and standards within a rehabilitation/recovery, harm-reduction focused model;
  • Ability to practice from a harm reduction approach;
  • Ability to build relationships and nurture an environment of quality, trauma-informed, and culturally safe care;
  • Effective communication and interpersonal skills;
  • Organizational, problem-solving, and teaching skills;
  • Judgment, good observation, and client assessment skills, tact, and empathy in working with addiction clients;
  • Ability to work collaboratively as part of an interdisciplinary team;
  • Promote positive change and independence;
  • Prioritize and organize workload;
  • Ability to carry out the duties of the position;
  • Operate a personal computer with a variety of Windows-based software;
  • Foster good public relations;
  • Operate related equipment;
