About the Role

The Abilities Management, Case Manager plays a crucial role in our People and Culture team, providing case management support and direction to employees and stakeholders for occupational and non-occupational injuries and illnesses. This position collaborates with internal stakeholders and interacts with healthcare professionals and insurers to ensure a safe and sustainable return to work for employees.

This role offers an exciting opportunity to coordinate case management activities, assess employee readiness for return to work, and develop individual plans to facilitate early and safe return to work. The successful candidate will have strong analytical and problem-solving skills, as well as excellent written and oral communication abilities.

Key Responsibilities
  1. Coordinate case management activities to support safe, timely, and sustainable return to work.
  2. Support employees through collaboration with health care providers, monitoring progress of employees, communicating, and collaborating with managers and union representatives.
  3. Assess employees' readiness for return to work based on medical and functional information.
  4. Develop individual plans to facilitate an early and safe return to work that meet employees' functional ability and individual needs.
  5. Document case activity and decisions in a timely, accurate, confidential manner.
  6. Promote best practices in disability management.
  7. Provide expert consultation, advice, and coaching to stakeholders and collaborate with leaders to resolve issues on complex cases.

This is a hybrid work opportunity with the expectation that the successful candidate will reside in Victoria and work out of the Victoria Transit Administration building at least two days per week.

About You

The ideal candidate will have experience applying best practices in disability case management, early intervention, and return-to-work planning. Strong problem-solving and analytical skills, as well as excellent written and oral communication abilities, are essential for success in this role.
