Job Title: Case Management Specialist
Job Summary: We are seeking a skilled Case Management Specialist to support individuals impacted by disruptive events. As a Case Management Specialist, you will work closely with clients to identify their needs, develop recovery plans, and provide access to relevant services.
Key Responsibilities:
- Support individuals in a confidential and empathetic setting, gathering information about their experiences and needs.
- Determine immediate, medium, and long-term needs, and assess relevant services that can support the individual.
- Provide appropriate services and solutions, including basic needs, referrals, financial assistance, recovery planning, and others.
- Facilitate inclusive access to CRC services using a person-centered approach, considering cultural, personal, and unique factors.
- Develop recovery plans that address individual needs, strengths, vulnerabilities, and capacities.
- Book follow-up meetings to check-in and ensure clients know their next steps.
- Provide referrals to existing CRC services and programs, or externally to service providers.
- Ensure case files are complete within EMIS, with supporting documents scanned and stored appropriately.
- Escalate compliance concerns, file irregularities, incidents, and fraud to the case management supervisor.
Requirements:
- College degree in a related field, and 1-3 years of experience working in customer service, case management, disaster recovery, health services, or a related field.
- Knowledge or experience in customer service, disaster recovery, emergency management, community development, social work, or psychology.
- Experience in emergency settings is an asset.
- Understanding of Red Cross role in emergency response and recovery.
- Experience using database systems, including EMIS, is an asset.
- Knowledge of Microsoft suite of tools, including Word, Excel, and SharePoint.
- Current First Aid certification or willingness to take training.
- Language requirements: must be able to read, write, and deliver services in English or French.
Working Conditions:
- The majority of the work is performed in a clean and comfortable environment.
- Potential for responding in disaster areas, as well as difficult and unstable conditions, and uncomfortable conditions.
- Ability to respond during non-traditional hours and long hours of work in a demanding context.
- Ability to respond in local and outside communities.
