Job Summary:
We are seeking a compassionate and dedicated Case Manager to join our team. As a Case Manager, you will be responsible for coordinating and managing the care of patients in various healthcare settings. Your role will involve assessing patient needs, developing care plans, and collaborating with healthcare professionals to ensure the best possible outcomes for patients. If you have experience in hospice care, acute care, or case management, we encourage you to apply.
**Duties**:
- Conduct comprehensive assessments of patients' physical, emotional, and social needs
- Develop individualized care plans based on patient assessments and goals
- Coordinate and facilitate communication between patients, families, and healthcare providers
- Advocate for patients' rights and ensure they receive appropriate care and services
- Monitor patient progress and adjust care plans as needed
- Collaborate with interdisciplinary teams to coordinate and manage patient care
- Provide education and support to patients and their families regarding their healthcare options
- Maintain accurate and up-to-date documentation of patient interactions and interventions
**Requirements**:
- Bachelor's degree in nursing or a related field (required)
- Experience in hospice care, acute care, or case management (preferred)
- Knowledge of medical terminology and healthcare practices
- Strong organizational skills and attention to detail
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Familiarity with hospital systems and electronic medical records (EMR)
- Experience working with pediatric patients (preferred)
**Job Type**: Casual
**Salary**: $25.00-$40.00 per hour
