**Purpose**:
Reporting to the Justice Program Manager, the Transitional Case Manager (TCM) is to facilitate and support the successful transition of forensic inpatients into the community. The TCM will work in partnership with the Mental Health & the Law Program of the North Bay Regional Health Centre. The worker will provide flexible, community-based services within a case management model advised by a recovery approach.
**Education and training**:
- Proof of successful completion of a post-secondary education in Social Services, Criminal Justice, Mental Health and Addiction or Nursing and proof of registration with the College of Nurses (if applicable)
**Requirements**:
- A negative Police Vulnerable Sector Check
- A valid class G driver’s license and proof of insurance
- Daily access to a reliable vehicle
- Current CPR certificate - level C or willingness to obtain
- Current Non-Violent Crisis Intervention (NVCI) training or willingness to obtain
- Current Applied Suicide Intervention Skills Training (ASIST) or willingness to obtain
- Current Safe Food Handling course or willingness to obtain
- Able to lift at a minimum 20 lbs. from squatting to standing position
- Bilingualism (English/French) is required
**Experience**:
- Minimum 2 years of experience working with vulnerable populations, specifically adults living with a serious and persistent mental illness and/or acquired brain injury (ABI) and/or Addictions and/or complex healthcare needs providing assessments and rehabilitation services
- Demonstrated knowledge of, and skills working in, the mental health field, forensics, Ontario Review Boards, psychosocial assessments, crisis intervention, community resources and the criminal justice system
- Experience working in a community setting and working knowledge of community resources
**Knowledge/Skills/Abilities**:
- Basic working knowledge of computer software
- Ability to work cooperatively and independently with mínimal supervision and as part of a team
- Effective problem solving and decision-making skills
- Proficiency in managing challenging behaviours
- Excellent interpersonal/verbal/written communication skills
- Ability to maintain confidentiality as per PHIPA (Personal Health Information Protection Act)
- Working knowledge of the Mental Health Justice System, Mental Health Act, Residential Tenancy Act and the Health Care Consent Act
- Knowledge/experience using the principles and practices of psychosocial rehabilitation
- Strong knowledge of mental illness, forensics, crisis intervention, life skills training, and knowledge of community resources
- Ability to provide education to clients and family members to better understand and cope with their mental illness including medication issues and relapse prevention
- Must exhibit sound judgment, tact and diplomacy and must at all times respect the confidentiality of both program and client information
