**This Part-Time Opportunity Awaits:**
We're seeking a skilled nurse for a Part-Time position in the Sundridge, ON area. The role involves working within a 2-week rotation, inclusive of weekends and on-call shifts.
**Your Responsibilities Will Include:**
- Developing and maintaining effective care plans in cooperation with Ontario Health @Home (OH@Home)
- Providing exceptional nursing services to patients in-person and virtually
- Communicating and liaising with patients, family members, and the interdisciplinary care team
- Maintaining a valid driver's license and vehicle, as regular travel is required
- Holding excellent English verbal and written communication skills, as well as a current immunization record

**CarePartners In Your Community:**
In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and relief in retirement homes and shared care settings. Through our Community Nursing Services outreach program, we've been organizing staff-led medical care and clinics in countries with poor access to healthcare since 2009.
**Estimated Salary:** $60,000 - $80,000 per year, depending on experience.
