Job Description

In this role, you will be responsible for ensuring a positive patient and family experience by effectively scheduling direct care providers in the community to meet the requirements of patient care plans. You'll be part of a team of professionals working diligently to provide consistent care while meeting the scheduling requirements of both patients and care providers.

You will use your excellent communication skills to communicate effectively with care providers, patients, and families when schedule challenges arise, and problem-solve effectively. Additionally, you will maintain accurate records of care provider availability and ensure dated notes are timely and clear, containing appropriate and required information.

Required Skills and Qualifications

  • Customer Service Experience: Strong customer service experience, with the ability to remain calm in stressful situations and empathize with the complexity and difficulty of each client or field staff situation.
  • Computer Skills: Proficiency in using scheduling software and other computer systems to fulfill patients' care plans.
  • Telephone Etiquette: Excellent telephone etiquette, with the ability to communicate effectively with patients, families, care providers, and referral sources.
  • Problem-Solving Abilities: Strong analytical problem-solving abilities, with the ability to frequently problem-solve and escalate concerns or risks as required.
  • Interpersonal Skills: Combine strong interpersonal skills with analytical problem-solving abilities to deliver excellent care coordination services.

Estimated Salary: $45,000 - $60,000 per year
