Job Summary
Mackenzie Health is seeking a highly skilled and experienced Nurse Practitioner to join our team. As an autonomous health professional, you will provide essential health services grounded in professional, ethical, and legal standards.
Key Responsibilities
- Provide comprehensive health services encompassing identification and management of chronic stable conditions, prevention of disease and injuries, health promotion and education, rehabilitation, continuity of care, and support services.
- Assess and provide services to clients, families, and communities.
- Work collaboratively with clients and other healthcare providers to provide high-quality patient-centered care.
- Develop and implement education initiatives for healthcare providers and community members.
- Participate in case management and leadership in clinical excellence.
Requirements
- Registered with the College of Nurses of Ontario as an Extended Class RN and a member in good standing.
- Minimum of 5 years of recent clinical experience in Medicine required.
- Membership in the Nurse Practitioner Association of Ontario preferred.
- National certification in Medical/Surgical or Gerontology Nursing from the Canadian Nurses Association required.
Preferred Qualifications
- Advanced clinical knowledge of current best practices in caring for medicine patients.
- Demonstrated knowledge and experience of theories guiding adult learning principles and principles guiding health promotion activities.
- Experience in case management.
- Embraces a philosophy of patient and family-centered care.
- Demonstrates leadership in clinical excellence.
