Nurse Practitioner Part-Time Contract Opportunity

Flemingdon Health Centre is seeking a skilled and dedicated Nurse Practitioner to join our interdisciplinary team in a part-time contract position. As a key member of our primary health care team, you will provide comprehensive care to our clients, working collaboratively to address the social determinants of health and improve access to health care services.
Responsibilities:- Provide comprehensive primary health care services, including assessments, diagnosis, treatment plans, and follow-up care.
- Coordinate client care within our health centre and with other healthcare agencies and specialists.
- Develop and implement health teaching and promotion programs with clients as part of clinical assessments and client care.
- Participate in quality improvement programs and contribute to the development and delivery of group programs or special presentations.
- Collaborate with our interdisciplinary team to provide comprehensive care to our clients.
- Registered Nurse with the College of Nurses of Ontario in the Extended Class.
- Baccalaureate in Nursing and graduate of a recognized educational program for primary care nurse practitioners.
- 2-3 years of experience in primary care or community nursing is an asset.
- Ability to work as a member of an interdisciplinary healthcare team, responsible and accountable for own practice.
- Excellent oral and written communication skills.
- Ability to work flexible hours, including evenings and weekends.
