Healthcare Coordinator Opportunity

At the Winnipeg Regional Health Authority, we are seeking a skilled and compassionate individual to fill the role of Case Coordinator. This position plays a crucial part in determining program eligibility and assessing the need for care at home or in a personal care home for elderly or infirm adults.
About the RoleThe successful candidate will be responsible for coordinating the delivery of various professional and non-professional services based on established guidelines. As a liaison, they will maintain communication with relevant agencies and facilities involved with the client group.
Key Responsibilities- Determine program eligibility and assess care needs related to Home Care
- Perform assessments and prioritize patterns/needs
- Establish appropriate discipline coordinators
- Develop and implement care plans, including service provision and evaluation criteria
- Implement and coordinate care plans, which may involve initiating medical rehabilitation and consultative services as necessary
In addition to these responsibilities, the successful candidate will also:
- Maintain current case counts and submit statistics properly
- Plan and organize work schedules effectively, managing caseload demands
- Utilize consultation and supervision
- Gather data regarding resources and resource needs related to caseload/community
- Participate in educational development when required, including new staff, students, and program-related education
To succeed in this role, you will require:
- A degree in a relevant field, such as BSW, OT, PT, BSc PN, RPN, BN, RN, SLP, RD, or related health/human service fields
- Valid driver's license and vehicle
- Licensure/registration as per professional designation
We are looking for individuals with:
- Two years of directly related community experience (preferable)
- Previous experience as a Case Coordinator in Community Health Services (preferable)
- Knowledge and experience with computers
- Effective oral and written communication skills
