Job Summary

The Clinical Forensic Social Worker - Community and Liaison Services will provide clinical services to individuals with complex mental health and substance use challenges. The successful candidate will work collaboratively with the treatment team to assess and develop treatment plans, provide community case coordination and advocacy services, and facilitate access to psychiatric and social services.

Key Responsibilities

  • Conduct initial intake and ongoing biopsychosocial assessments to identify client needs and develop integrated treatment plans.
  • Provide community case coordination and advocacy services to clients and their families, including referrals to other health and social services.
  • Facilitate access to psychiatric and social services through referrals, site visits, and home visits as required.
  • Collaborate with the treatment team to assess and develop treatment plans, and provide written reports and summaries as required.
  • Monitor client compliance with treatment plans and initiate direct breach proceedings as necessary.

Requirements

  • Bachelor's degree in Social Work from an accredited School of Social Work.
  • Two (2) years recent, related experience working in a mental health setting.
  • Effective registration with the British Columbia College of Social Workers (BCCSW) is required.
  • Valid BC Drivers License. Local area travel may require the use of a personal vehicle.
  • Current Cardiopulmonary Resuscitation (CPR) Certificate.

Preferred Qualifications

  • Broad knowledge of social work theory and forensic psychiatric social work practice.
  • Broad knowledge of BCCSW professional practice standards.
  • Broad knowledge of other facilities and community resources.
  • Broad knowledge of other health care disciplines and their role in client care.
  • Broad knowledge of Bill C30 and the Provincial Court system.
  • Broad knowledge of psychopharmacology (indications and side-effects).
