Forensic Community and Liaison Social Worker

Job Summary
This is a challenging and rewarding opportunity to work as a Forensic Community and Liaison Social Worker with the Provincial Health Services Authority (PHSA). As a member of our team, you will be responsible for providing clinical services to individuals with complex mental health and substance use challenges.
Key Responsibilities
- Conduct comprehensive assessments of clients to identify their needs and develop individualized treatment plans.
- Provide case coordination and advocacy services to clients and their families, ensuring seamless transitions between healthcare services.
- Collaborate with multidisciplinary teams to provide comprehensive care, including psychiatric and social services.
- Develop and maintain relationships with community resources and service providers to ensure access to necessary support services.
- Provide education and support to clients and their families on mental health and substance use issues, as well as on available treatment options.
- Participate in pre-release planning conferences and consult with clinical teams to determine client needs and develop strategies for community reintegration.
Requirements
- Bachelor's degree in Social Work from an accredited institution.
- Two years of recent experience working in a mental health setting.
- Registration with the British Columbia College of Social Workers (BCCSW) is required.
- Valid BC Driver's License and access to a reliable vehicle.
- Current CPR Certificate.
Preferred Qualifications
- Experience working with Indigenous populations and knowledge of Indigenous-specific anti-racism initiatives.
- Knowledge of social, economic, political, and historical realities of settler colonialism on Indigenous Peoples.
- Familiarity with addressing Indigenous-specific anti-racism, anti-racism, and Indigenous Cultural Safety.
