Position: Forensic Community Nurse

Overview:

As a vital member of the BC Mental Health & Substance Use Services team, you will engage in comprehensive nursing practices aimed at supporting individuals with complex mental health challenges. Your role will encompass a variety of responsibilities designed to enhance patient care and community integration.

Key Responsibilities:

  • Perform thorough initial assessments and ongoing biopsychosocial evaluations, including mental status checks and physical health assessments. Develop and implement an Integrated Treatment Plan (ITP) based on clinical evaluations and risk assessments.
  • Coordinate community-based case management and advocacy services, ensuring effective communication and collaboration with mental health teams, medical professionals, and social service agencies to facilitate continuity of care.
  • Maintain precise and comprehensive documentation in accordance with regulatory standards, ensuring that all client interactions and treatment plans are well-documented to support clinical decisions.
  • Provide essential nursing services and therapeutic support, addressing immediate mental health needs, administering medications, and offering guidance on treatment options and resources available to clients.
  • Facilitate access to psychiatric and social services, conducting home visits and community outreach to assist clients in their reintegration into society.

Qualifications:

  • Graduation from an accredited School of Nursing with current registration as a Registered Nurse or Registered Psychiatric Nurse.
  • A minimum of two years of relevant experience working with individuals facing severe and persistent psychiatric conditions.
  • Valid driver's license for local travel as required.

Skills and Knowledge:

  • Extensive understanding of nursing theories and forensic psychiatric nursing practices.
  • Familiarity with the standards of professional practice and ethical guidelines set forth by the British Columbia College of Nurses & Midwives.
  • Knowledge of community resources and interdisciplinary roles in client care.
  • Awareness of the implications of Bill C30 and the Provincial Court system.
  • Understanding of psychopharmacology, including medication indications and potential side effects.
  • Commitment to the principles of reconciliation and cultural safety, particularly concerning Indigenous communities.
