Job Summary:- This position involves assessing patients' needs and developing individualized care plans to ensure optimal health outcomes.
- Assessing patient needs through thorough evaluations and medical histories.
- Developing and implementing personalized care plans in collaboration with healthcare professionals.
- Administering medications and treatments as prescribed by physicians.
- Monitoring patients' vital signs and reporting any changes in condition to the medical team.
- Collaborating with healthcare professionals to ensure coordinated care and achieve positive patient outcomes.
- Providing education and support to patients and their families on healthy lifestyle habits and disease management.
- A valid RPN license in Canada.
- Completion of a recognized Practical Nursing program.
- Previous experience in a healthcare setting is preferred, with a strong focus on patient care and coordination.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- The ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities with ease.
- A competitive salary package of $83,000 - $110,000 per year, depending on experience.
