Caregiver Support Specialist

About the RoleThis position requires a highly organized and empathetic individual who can effectively communicate with clients, families, and healthcare professionals to ensure seamless care delivery. As a Caregiver Support Specialist, you will be responsible for providing personal support and homemaking services to clients, assessing their needs, and developing tailored care plans.
Responsibilities- Provide personalized support and care to clients in their own homes, including light housekeeping, meal preparation, and personal care.
- Assess client needs and develop comprehensive care plans in collaboration with healthcare professionals.
- Communicate effectively with clients, families, and healthcare teams to ensure coordinated care delivery.
- Contribute to the development and implementation of care plans, ensuring safety, liability, and financial considerations.
- Report changes in client needs or home situations to supervisors and healthcare teams.
To be successful in this role, you must possess:
- A high school diploma; secondary education preferred.
- Current Standard First Aid Certificate or CPR Level C Certificate (if applicable).
- Experience working with elderly or ill individuals, children, or households.
- Strong communication skills, both verbal and written.
- Ability to work effectively in a multidisciplinary team or independently as required.
This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in this role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
