About the Role

As a Psychosocial Specialist with Fraser Health's Sexual Health and Blood Borne Infections team, you will play a pivotal role in providing clinical and consultative case management services to individuals living with HIV. Your expertise will be essential in facilitating communication and decision-making around client care, identifying gaps in services, and collaborating with community agencies to improve client outcomes.

Key Responsibilities
  • Provide advanced psychosocial assessments, counseling, referrals, and advocacy for clients and families living with HIV.
  • Crisis intervention and problem-solving support for high-risk behaviors and biopsychosocial difficulties.
  • Identify and refer clients to appropriate resources and services based on established protocols.
  • Coordinate and monitor individualized care plans that address client needs, ensuring effective engagement along the continuum of care.
  • Establish and maintain relationships with community social service agencies, facilitating communication and decision-making around client care.

Qualifications
  • Bachelor's degree in Social Work from an approved school.
  • Two years of recent experience in complex healthcare environments, including one year working with marginalized clients with multiple health issues.
