Nurse Specialist Hospice Care

About the Role
As a member of our multidisciplinary team, you will provide direct nursing care to patients in the Hospice Residence at assigned locations. You will assess physical, emotional, spiritual, and psychosocial needs, plan, document, and implement care plans, and evaluate their effectiveness.
Key Responsibilities
- Assess and plan nursing care for assigned patients, identifying actual or potential problems and strengths.
- Implement and evaluate nursing care, collecting information from various sources to enable appropriate care.
- Communicate and consult with healthcare team members about patient care plans, participating in interdisciplinary conferences and explaining plans to patients and families.
- Act as an advocate to protect and promote patients' rights to autonomy, respect, privacy, dignity, and access to information.
- Document timely and accurate reports of relevant observations, including patient and family teaching and evaluation of nursing care.
- Participate in nursing, hospital committees, and councils to improve patient care and work environment.
- Provide information and act as a preceptor for nursing staff and students, identifying self-learning needs and attending educational programs to maintain clinical competency.
- Participate in quality improvement activities by identifying patient care issues and collecting data, evaluating patient care programs, techniques, standards, and procedures.
Qualifications
Graduation from an approved School of Nursing and/or Psychiatric Nursing, with two years' recent related medical nursing experience in an acute care facility, including one year recent related experience in a hospice/palliative care environment, or an equivalent combination of education, training, and experience. Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Skills and Abilities
Knowledge of nursing practice as it pertains to hospice palliative/residence patients, knowledge of other healthcare professions and their contribution to the multidisciplinary team, ability to work independently and as a member of a team, ability to communicate effectively both verbally and in writing, ability to organize and prioritize work in a dynamic environment, ability to develop and maintain rapport with patients, families, and caregivers, and physical ability to perform the duties of the position.
