Forensic Community and Liaison Social Worker Role

BC Mental Health and Substance Use Services

Vancouver, BC

Key Responsibilities

  • Conduct comprehensive biopsychosocial assessments to identify client needs and develop integrated treatment plans.
  • Provide community case coordination and advocacy services, collaborating with healthcare providers and community resources to ensure seamless care.
  • Maintain accurate and concise documentation of client information, adhering to regulatory requirements and professional standards.
  • Offer clinical services, information, and support to clients, including education on mental illness, treatment options, and medication management.
  • Facilitate access to psychiatric and social services through referrals, site visits, and home visits as needed.
  • Participate in pre-release planning conferences and consult with the clinical treatment team to determine client needs and safety requirements.

Requirements

  • Bachelor's degree in Social Work from an accredited institution.
  • Two years of recent experience working in a mental health setting.
  • Full registration with the British Columbia College of Social Workers (BCCSW) is required.
  • Valid BC Driver's License and access to a reliable vehicle.
  • Current Cardiopulmonary Resuscitation (CPR) Certificate.
