Job Summary
We are seeking a highly skilled Forensic Community and Liaison Social Worker to join our team at BC Mental Health and Substance Use Services. As a key member of our team, you will be responsible for providing clinical services, information, and support to clients with complex mental health and substance use challenges.
Key Responsibilities
- Conduct initial intake and ongoing biopsychosocial assessments to identify client needs and develop integrated treatment plans.
- Provide community case coordination and advocacy services, collaborating with other health and service providers to ensure comprehensive care.
- Maintain accurate and concise documentation of client information, adhering to FPSC policy standards and regulatory requirements.
- Provide written reports and summaries as per FPSC policy, standards, and guidelines.
- Facilitate access to psychiatric and social services through referrals, site visits, and home visits as per the ITP and FPSC policy.
- Consult with the clinical treatment team to determine client needs and develop strategies for community reintegration and prosocial development.
Requirements
- Bachelor's degree in Social Work from an accredited School of Social Work.
- Two (2) years recent, related experience working in a mental health setting.
- Effective September 1, 2013, full registration with the British Columbia College of Social Workers (BCCSW) is required.
- Valid BC Drivers License. Local area travel may require the use of a personal vehicle.
- Current Cardiopulmonary Resuscitation (CPR) Certificate.
