Job Title: Forensic Community and Liaison Social Worker

We are seeking a highly skilled and experienced Forensic Community and Liaison Social Worker to join our team at BC Mental Health and Substance Use Services. As a key member of our team, you will work closely with clients, families, and other healthcare professionals to provide comprehensive social work services in a forensic mental health setting.

Key Responsibilities:
  • Conduct initial intake and ongoing biopsychosocial assessments to identify client needs and develop integrated treatment plans.
  • Provide community case coordination and advocacy services to support clients in accessing community resources and services.
  • Maintain accurate and concise documentation of client information and provide written reports as required.
  • Collaborate with other healthcare professionals to provide comprehensive care and support to clients.
  • Facilitate access to psychiatric and social services through referrals and site visits.
Requirements:
  • Bachelor's degree in Social Work from an accredited School of Social Work.
  • Two years recent, related experience working in a mental health setting.
  • Full registration with the British Columbia College of Social Workers (BCCSW).
  • Valid BC Drivers License.
  • Current Cardiopulmonary Resuscitation (CPR) Certificate.
