Mental Health Community Case Manager

Health Careers in Sask is seeking a skilled Mental Health Community Case Manager to join our team. As a Community Mental Health Nurse, you will provide a range of intake assessment, intervention, navigation services and clinical services to individuals with mental health and addiction issues and support to their families. You will work within an interdisciplinary mental health and addiction team, under the direction of the Outpatient Services Manager.
Key Responsibilities:
- Provide intake assessment, intervention, navigation services and clinical services to individuals with mental health and addiction issues and support to their families.
- Work within an interdisciplinary mental health and addiction team, under the direction of the Outpatient Services Manager.
- Serve as an important link between Intermediate Care Services and the services provided by the Primary Health Care System and community.
Requirements:
- 2 years working experience in community mental health and addictions services.
- Psychiatric Nursing diploma (DPN).
- Ability to design, initiate and conduct training for other professionals.
- Ability to work with a variety of individuals presenting with complex issues.
- Competency in psychiatric nursing assessments, care planning & treatment.
- Ability to provide public/educational presentations.
- Knowledge and practical application of the Mental Health Services Act.
- Ability to enter and retrieve data in computer database systems.
- Ability to work independently and as a member of a multi-disciplinary team.
- Ability to meet multiple demands, manage complex clinical issues and workload.
