Mobile Wellness Client Care Navigator

**Summary**:
The Client Care Navigator is a member of the mental health and addiction team that contributes to the effective delivery of mental health and addiction services with clients referred to the Mobile Wellness Clinic. The Client Care Navigator is the main point of contact for the Mobile Wellness Clinic and will collaborate in triage and screening of all individuals seeking service and determine type of services needed. The Mobile Wellness Clinic serves all residents within the Manitoulin District.
**Job Duties**:
**1. Providing entry/intake services**:
- Monitor and ensure timely processing of incoming referrals/service requests
- Perform intakes with individuals seeking service using program intake forms
- Assign individuals to members of the clinical mental health and addiction team based on individual’s needs and availability of clinicians
- Managing the individual’s expectations regarding availability and timeframe for accessing programs and services
- Manage bookings and scheduling of appointments for psychiatry clinics
- Provide support to clients during psychiatry clinics and intervene in situational crises and create safety plans as needed.
- Remain current on knowledge of existing and surrounding MH&A services and referral networks related to such services
- System navigation and making referrals to appropriate programs and services when necessary, internally and externally
- Maintaining a good working relationship and ongoing communication with referral sources and community agency representatives
- Delivering community presentations regarding mental health and addiction services and referral and intake processes
- Emotional support/supportive counseling to clients seeking services and building trust and rapport with clients
- Provide services on the mobile wellness van as needed and directed by manager
- Maintaining all required documentation and data entry in the execution of their clinical practice in a timely manner
- Maintaining a current knowledge and skill base relative to the agencies clinical practice through identifying and participating in ongoing professional development activities
- Other duties as assigned
**Requirements**:
- BSW or BA in a social work/health related discipline or Diploma in a health-related discipline with minimum 3 years experience in a mental health and addiction setting will also be considered
- Knowledge of traditional health practices
- Knowledge of current Best Practices in the treatment of substance use
- Additional trainings and/or certificates that focused on opiate use and the risks associated with use considered an asset
- Additional trainings around crisis intervention, prevention and de-escalation considered an asset
- Demonstrated skills in system navigation, treatment planning, and case management
- Vulnerable Sector Check
- Valid Ontario Class G drivers license and access to a reliable vehicle with appropriate insurance
- Supportive and knowledgeable of both traditional and western models of health care;
- Prior experience utilizing medical scheduling software an asset, preferably with an EMR (ideally PS Suites)
- Extensive computer literacy
- Knowledge of various Indigenous and mainstream community organizations, service providers, and resources
- Exceptional attention to detail and accuracy
- Ability to manage confidential and sensitive information
