Job DescriptionThis role plays a crucial part in our mission to deliver high-quality patient care. As a Care Coordinator, you will assess patient needs, determine eligibility for services, and develop individual care plans. You will link patients with service providers, coordinate and monitor care plan delivery, and establish helping relationships with patients and their families.

To be successful in this role, you must possess excellent interpersonal, communication, assessment, problem-solving, and decision-making skills. You should also have knowledge of the healthcare delivery system and community resources. A valid driver's license and access to a reliable vehicle are required.
