Client Intake & Assessment Facilitator

PACE Independent Living is seeking a skilled and compassionate Client Intake & Assessment Facilitator to join our Client Resource Team. As a key member of our team, you will be responsible for facilitating client intake, transfer, and discharge, as well as conducting care assessments and case management to support clients in living independently and promoting quality of life.

Key Responsibilities:
  • Facilitate client intake, transfer, and discharge processes, including short-listing clients for interviews, conducting assessments, and developing client case notes.
  • Conduct care assessments using various tools, such as the Barthel Index, Mayo Portland, and InterRAI, to determine client eligibility and suitability for services.
  • Build relationships with healthcare providers, vendors, and community partners to integrate services and promote continuity and quality of care.
  • Collaborate with interdisciplinary teams to navigate access to required services and support clients in navigating the healthcare system.
Requirements:
  • Post-secondary degree or diploma in Social Work, Social Services, or a related discipline.
  • Minimum of three years' experience in a human services organization, preferably in a unionized setting with adults in the disability sector.
  • Certified InterRAI Assessor designation.
  • Excellent communication and collaboration skills, with the ability to work in a changing environment.

  • Competitive salary ($61,955 to $69,731).

  • Hybrid work arrangement, with three days per week at Head Office and two days per week remote.
  • Regular travel to other program sites and client homes in the GTA.
