Job Summary:

The Baycrest Inpatient Low Tolerance Long Duration (LTLD) Rehabilitation Unit is seeking a skilled Registered Practical Nurse (RPN) to join our interdisciplinary care team. As a key member of our team, you will play a vital role in providing high-quality patient care and supporting our patients in achieving their maximum physical and functional goals.

Responsibilities:
  • Develop and implement individualized care plans, incorporating nursing interventions and treatments within the RPN's scope of practice.
  • Collaborate with the interdisciplinary care team to provide client-centered care, addressing personal care, activation, recreation, and spiritual needs of patients.
  • Work closely with Registered Nurses to develop and implement care plans, ensuring seamless care transitions.
Qualifications:
  • Current registration in good standing with the College of Nurses of Ontario (CNO) is required.
  • Current Basic Cardiac Life Support (BCLS) certificate, Level C - Automated External Device (AED) is required.
  • Previous geriatric rehabilitation experience is preferred.
  • Excellent interpersonal, communication, and problem-solving skills are essential.
  • Basic computer skills, including Meditech, Microsoft Word, and Excel, are preferred.
