Job Summary

We are seeking a highly skilled and compassionate Senior Client Support Specialist to join our dynamic team at Reconnect Health Services. About Us

Reconnect Health Services is a thriving not-for-profit healthcare provider dedicated to delivering exceptional client-centered care in the west end of Toronto. Our services include supports for seniors, caregivers, and individuals living with mental health and addictions concerns.

Responsibilities
  • Provide ongoing case management support and service coordination to clients, developing and maintaining personalized care plans until discharged from our services.
  • Initiate intake interviews and conduct needs assessments to identify client requirements.
  • Work closely with internal and external stakeholders, including family members/caregivers, healthcare providers, and community agencies, to coordinate effective service delivery.
  • Maintain accurate records and documentation, adhering to established policies and procedures.
Requirements
  • College Diploma/University degree in social services or related field (BSW or MSW an asset).
  • Minimum three years related experience in seniors case management and care coordination support.
  • Extensive knowledge of seniors issues and services, as well as experience working with clients with age-related mental and/or physical health issues.
  • Excellent verbal and written communication skills, with demonstrated case management and referral skills.
  • Ability to work independently and effectively as part of a multidisciplinary team.
Benefits

We offer a competitive salary of $48,934 per year, plus benefits including vacation time, paid sick days, and health and RRSP benefits. This is a unionized position, and we value innovation, compassion, and hard work.
