Home and Community Care Support Services is seeking a skilled Geriatric Care Specialist to join our team. As a Geriatric Care Specialist, you will play a vital role in providing high-quality care to adults in our community.

Key Responsibilities:
  • Conduct comprehensive geriatric assessments using the PIECES framework and supporting geriatric assessments.
  • Liaise with care teams to develop and implement behavioral care plans.
  • Collaborate with healthcare professionals to coordinate care and ensure seamless transitions.
  • Provide education and support to clients and caregivers on behavioral care strategies.
  • Document assessments and care plans in the LHIN CHRIS database and other relevant systems.
Requirements:
  • Registered Nurse (BScN or diploma) in good standing with the College of Nurses of Ontario.
  • Minimum of two years of experience in geriatric care or a related field.
  • Knowledge of geriatric care principles, including assessment, behavior, outcomes, and evaluation.
  • Ability to work collaboratively with healthcare teams and communicate effectively with clients and caregivers.
