Job Title: Geriatric Behaviour Specialist

Home and Community Care Support Services is seeking a skilled Geriatric Behaviour Specialist to join our team. As a Registered Nurse, you will play a vital role in providing high-quality care to adults in our community.

Key Responsibilities:
  • Conduct comprehensive geriatric behaviour assessments using the PIECES framework and supporting geriatric assessments.
  • Liaise with care teams to develop and implement behavioural care plans.
  • Collaborate with healthcare professionals to coordinate care and ensure seamless transitions.
  • Provide education and support to clients and caregivers on behaviour management.
  • Document assessments and care plans in our electronic health record system.
Requirements:
  • Registered Nurse (BScN or diploma) in good standing with the College of Nurses of Ontario.
  • Minimum of two years of experience in geriatric care or mental health settings.
  • Knowledge of geriatric behaviour, dementia, and mental health.
  • Ability to work independently and as part of a team.
