Job Title: Registered Practical Nurse - Geriatric Care Specialist

Join our team of dedicated healthcare professionals at William Osler Health System as a Registered Practical Nurse (RPN) in the Acute Care for the Elderly (ACE) Unit. As a key member of our inter-professional team, you will provide high-quality patient-focused care to patients with complex geriatric needs.

Key Responsibilities:
  • Implement the Butterfly model of care, an interactive emotions-based model that reduces or eliminates behaviors associated with dementia and/or delirium.
  • Work collaboratively with the Butterfly model team to evaluate and sustain the foundational components of the model.
  • Support the Butterfly concept and model by attending education related to the model of care.
  • Inspire and lead others to effectively implement the Butterfly model through role modelling, coaching, and communication.
  • Participate in change methodology to support and sustain the change to an emotion-based model of care.
  • Provide safe nursing practice in all aspects of the nursing process: assessment, planning, implementation, and evaluation of patient care.
  • Comply with policies and procedures associated with observations.
  • Provide professional, patient-centred care to ensure patient safety and physiological and psychological well-being.
  • Practice according to the infection control and prevention measures.
  • Respond to emergency situations by recognizing changes in patient's health status and consulting appropriately when necessary using transfer of accountability principles.
  • Provide health teaching to patients and families, including thorough discharge instructions to ensure continuity of care in the community.
Qualifications:
  • Experience within the last five years with a minimum of two years in acute medicine required.
  • Experience working with the geriatric and dementia patient in an acute care setting within the last two years required.
  • Comprehensive understanding of geriatric syndromes, delirium, and dementia.
  • Geriatric certificate enrollment required.
  • Gentle persuasion approach required.
  • PIECES enrollment preferred.
  • NICHE geriatric certificate preferred.
  • A passion for leading dementia care.
  • Up-to-date and evidence-based knowledge and skill in dementia care.
  • Your priority is the wellbeing of those patients on the unit, not only tasks.
  • You believe that emotions matter most.
  • Demonstrated experience in care of patients with TPN, NG tubes, chest tubes, tracheostomy, wound care, enteral feeds, IV therapy, blood collection, and isolation.
Core Competencies:
  • Current registration, and in good standing, with the College of Nurses of Ontario.
  • Sound knowledge of nursing process, theory, and responsibilities to the nursing profession.
  • BCLS required.
  • Medication administration certificate and patient assessment course required for RPNs graduating prior to 2005.
  • Demonstrated excellent interpersonal, communication, problem-solving, conflict-resolution, and decision-making skills.
  • Demonstrated ability to work collaboratively as a member of an inter-professional team.
  • Demonstrated knowledge of patient and family-centred care.
  • Demonstrated experience with patient and family health teaching required.
  • Demonstrated leadership and facilitation skills required.
  • Demonstrated excellent customer service, organizational, and critical-thinking skills required.
  • Demonstrated commitment to continuous learning and professional development as it applies to the profession and related field.
  • Demonstrated ability to work in a stressful environment.
  • Exemplary clinical practice within an inter-professional team to develop appropriate strategies for patient care.
  • Ability to prioritize patient care needs and make autonomous decisions.
  • Excellent physical assessment skills required.
  • Ability to promote a healthy work environment and practice excellent client safety guidelines.
  • Knowledge and understanding of the Occupational Health and Safety Act.
  • Must demonstrate Osler's Values of Respect, Excellence, Service, Compassion, Innovation, and Collaboration.
  • May be required to travel and work between all Osler sites.
  • Excellent attendance, punctuality, and work record.
Additional Information:

Hours: Currently Days/Evening, 8 hour shifts including weekends and statutory holidays (subject to change in accordance with operational requirements).

Salary:
Minimum: $35.29 per hour
Maximum: $38.16 per hour
