Job Title: Social Worker - Cancer Care Specialist

North York General Hospital is seeking a highly skilled Social Worker to join our Cancer Care team. As a Social Worker, you will be a vital member of our multidisciplinary team, providing comprehensive support to patients and families navigating cancer care.

Key Responsibilities:
  • Conduct thorough assessments of patients' emotional, physical, and social needs
  • Develop and implement individualized care plans to address patients' needs and concerns
  • Provide counseling and support to patients and families, promoting patient-centered care
  • Collaborate with healthcare teams to ensure seamless care transitions and effective discharge planning
  • Stay up-to-date with community resources and advocate for patients' needs
Requirements:
  • Master of Social Work (MSW) or equivalent
  • Current registration with the Ontario College of Social Workers and Social Service Workers
  • 1-2 years of experience in palliative and cancer care, with knowledge of oncological diseases and multifaceted care plans
