Job Title: Social Worker, Paediatric Complex Care Clinic
Job Summary:
We are seeking a skilled Social Worker to join our Paediatric Complex Care Clinic team. As a Social Worker, you will provide appropriate interventions to patients and families through individual, family, and group treatment modalities. You will coordinate the utilization of resources to meet patients and families ongoing psychosocial and continuing care needs.
Key Responsibilities:
- Provide supportive counselling to parents to assist them in coping with the care needs of their child, the diagnosis of chronic conditions, and the impact of the complex care needs on their family.
- Coordinate the utilization of resources to meet patients and families ongoing psychosocial and continuing care needs.
- Participate in and/or facilitate educational opportunities for social work students and the paediatric healthcare team.
- Maintain excellent knowledge of, and close liaison, with hospital & community resources.
Qualifications:
- Master of Social Work degree required.
- Current registration with the Ontario College of Social Workers & Social Service Worker.
- Recent Paediatrics hospital experience.
- Recent experience working with children's aid society.
- Recent experience with clients with post-partum depression.
