Job Summary:
We are seeking a skilled Clinical Social Worker to join our team. The successful candidate will be responsible for assessing patients, implementing intervention plans, and providing clinical teaching to students. This is an exciting opportunity for a motivated and compassionate individual to make a difference in the lives of our patients.
Key Responsibilities:
- Assess patients to evaluate their needs and develop treatment plans
- Implement interventions to facilitate positive change and improve patient outcomes
- Provide clinical teaching and supervision to students
- Liaise with community agencies to ensure seamless care
- Complete applications for Alternate Level of Care facilities
Requirements:
- Master's degree in Social Work
- Registration with the Ontario College of Social Workers and Social Service Workers
- Membership in the Ontario Association of Social Workers (preferred)
- Three years of experience working with an inter-professional team in a hospital setting (preferred)
- Excellent verbal and written communication skills
- Excellent interpersonal, time management, and organizational skills
- Excellent problem-solving skills
