Job Summary

We are seeking a Senior Community Health Care Manager to join our team. As a key member of our management team, you will be responsible for leading a team of healthcare professionals and overseeing the delivery of in-home care services.

Key Responsibilities
  • Develop comprehensive care plans for clients using a holistic approach, taking into account their unique needs and goals.
  • Direct and monitor the delivery of all in-home services, ensuring that clients receive the highest quality care.
  • Hire and train new staff members, providing ongoing coaching and performance management.
  • Manage client expectations, ensuring a client-centered approach to care planning.
  • Adapt care to changing client needs, including managing acute illness and other immediate challenges.
  • Ensure shift coverage for callouts and sick calls, providing direct hands-on care when necessary.
Requirements
  • A current and valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) registration with the British Columbia College of Nurses & Midwives.
  • Minimum of five years of recent experience in a community setting.
  • Minimum of two years of recent experience leading/managing others.
  • Experience working with the geriatric population and in community healthcare is strongly preferred.
  • A reliable vehicle and willingness to travel to visit clients within the community.
What We Offer
