Job Title: Nurse Case Manager
Job Summary:
Nurse Case Managers play a vital role in providing comprehensive care to patients with complex health issues. As a key member of the Bayshore HealthCare team, you will be responsible for assessing, organizing, and evaluating patient care needs, developing personalized care plans, and collaborating with healthcare professionals to ensure seamless care coordination.
Key Responsibilities:
- Conduct comprehensive assessments of patient care needs, identifying areas for improvement and developing targeted care plans.
- Collaborate with healthcare professionals to ensure timely and effective care coordination, including referrals to specialty services and monitoring patient follow-ups.
- Provide education and support to patients and families, empowering them to manage their health and make informed decisions.
- Develop and maintain partnerships with community organizations to expand resources and improve patient outcomes.
- Participate in multidisciplinary team meetings to share patient progress and provide recommendations for care planning.
Requirements:
- Baccalaureate Degree in Nursing from a recognized university.
- Current license to practice from a provincial or territorial College of Nurses.
- Minimum 3 years of experience in nursing practice, with a focus on case management and patient education.
