About the Role

CarePartners is seeking a compassionate and dedicated Home Support Worker to join our team. As a Home Support Worker, you will provide professional and personalized care to patients in their homes and residences, enabling them to live life to the fullest with dignity and independence.

Key Responsibilities

  • Assist with activities of daily living, including bathing, showering, personal hygiene, dressing, meal preparation, transferring/repositioning, light housekeeping, bowel/bladder care, and socializing and companionship.
  • Provide emotional support and companionship to patients and their families.
  • Work collaboratively with healthcare professionals to develop and implement personalized care plans.
  • Maintain accurate and confidential records of patient care and progress.
