Mental Health Support Worker, Provincial Assessment Centre

As a Mental Health Support Worker, you will play a vital role in our team, working closely with clients, families, and healthcare professionals to provide compassionate and effective support.
Key Responsibilities:
- Monitor clients' progress, observing behaviors and symptoms, and documenting observations in accordance with standard procedures.
- Participate in the admission, case planning, and discharge process, conducting interviews with clients and families, gathering relevant history and documentation, and attending client case meetings.
- Establish rapport with clients, providing emotional support and counseling, and assisting with problem-solving and daily living activities.
- Coordinate services with medical professionals and community resources, acting as an advocate for clients and providing information on available services.
- Plan, organize, and facilitate groups, such as sports and exercise, social support groups, and social activities, tailored to the needs of clients.
- Prepare and maintain client charts, entering information into computerized databases and completing standard forms as required.
Requirements:
- Diploma from a recognized Social Services Support Worker educational program or equivalent combination of education, training, and experience.
- Valid driver's license and local area travel requires the use of a personal vehicle.
- Knowledge of the principles of recovery in mental illness, community, social, and health resources, behavior management, mental health issues, and treatment.
- Ability to establish and maintain rapport with clients, observe and recognize changes in clients, and deal effectively with conflict situations.
- Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada and knowledge of social, economic, political, and historical realities impacting indigenous communities.
