About the Role

Fraser Health is seeking a highly skilled Registered Nurse/Registered Psychiatric Nurse to join our team as a Community Mental Health Specialist. In this role, you will provide treatment and care management services to individuals with mental illness and concurrent disorders, working collaboratively with an interdisciplinary team-based model of care.

Key Responsibilities
  1. Client Care Coordination: Collaborate with the interprofessional care team to coordinate care for clients, perform intake functions, conduct mental health assessments, and develop comprehensive treatment plans.
  2. Nursing Care Planning and Delivery: Plan and provide direct nursing care to clients, including reviewing client/family history, conducting physical/psychosocial assessments, and implementing treatment modalities such as ADLs, wound care, and medication administration.
  3. Therapeutic Relationships: Establish and maintain therapeutic relationships with clients and their informal support network, engaging stakeholders in treatment planning and evaluation, and promoting client self-determination and independence.
  4. Treatment Services: Provide treatment services, including crisis intervention, individual counseling, and education, using a variety of theoretical models and approaches.
  5. Case Conferences and Collaboration: Participate in case conferences and meetings with clients, family, service providers, and members of the interprofessional team to identify client mental health issues, coordinate care, and ensure a coordinated approach to treatment.
  6. Client Advocacy: Act as a client advocate to protect and promote clients' rights to autonomy, respect, privacy, dignity, and access to information.
  7. Interprofessional Collaboration: Consult and collaborate with members of the interprofessional care team, including MH&SUS Physicians, Primary Care Providers, and other healthcare professionals/providers, to identify and resolve issues, adjust care plans, and ensure continuity of care.
  8. Extended Leave Monitoring: Monitor Extended Leave as required under the Mental Health Act, reviewing conditions, determining client compliance, and tracking renewal of certification due dates.
  9. Client Access and Support: Aid clients in accessing services, such as medical appointments, hospitals, and social programs, by providing healthcare information, contacting relevant programs/services, and assisting with processing forms and daily living tasks.
  10. Documentation and Safety: Document assessments, care plans, observations, interventions, client/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures, and maintain a safe environment for clients, visitors, and employees.
  11. Professional Development: Identify own learning needs/goals, review these with the Team Coordinator, and engage in activities to meet these needs/goals, and participate in research and quality improvement activities.
Qualifications

Education, Training & Experience:

  • Graduation from an approved School of Nursing or Psychiatric Nursing.
  • Two (2) years recent related experience providing treatment services to individuals with mental health and/or concurrent disorders, or an equivalent combination of education, training, and experience.
  • May require completion of a substance use certificate program.
  • May require eligibility for completion of the BC Provincial Opiate Addiction Treatment Support Program course and approved related preceptorship.

Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Valid Class 5 BC Driver's License and access to a personal vehicle with appropriate insurance coverage.
