We are seeking a skilled Social Worker to join our multidisciplinary team at Foundation Health Centre, a locally owned and operated healthcare clinic in downtown Fredericton. Our clinic provides a supportive environment for patients to receive comprehensive care.

Key Responsibilities:
  • Provide high-quality social work services to patients and their families
  • Collaborate with healthcare professionals to develop and implement patient care plans
  • Conduct assessments and interventions to address social determinants of health
  • Participate in team meetings and contribute to the development of policies and procedures
Requirements:
  • Master's degree in Social Work or related field
  • Current registration with the New Brunswick College of Social Workers
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
