Job Title: Full Time Program Services Caseworker

About the Role:

We are seeking a highly motivated and compassionate individual to support clients and the day-to-day operations of our residential facility. As a Program Services Caseworker, you will be responsible for:

Key Responsibilities:
  • Maintaining an active caseload and providing case management services to clients.
  • Completing intake and assessments, creating service plans, and providing clinical interventions for assigned clients.
  • Recording activities and incidents occurring during shifts in a factual and concise manner using Logbook and/or Incident Occurrence Reports.
  • Developing a network and making referrals to existing services and resources in the community.
  • Maintaining personal HIFIS account and using HIFIS system for all aspects of client case management.
  • Performing administrative tasks, including reporting monthly performance statistics and completing daily front-line worker responsibility logs and bed board charts.
Working Conditions:
  • This is a permanent full-time position with shift schedules.
  • PSCW staff members work onsite at Belinda's Place.
  • Minimal travel required.
Qualifications and Education Requirements:
  • College diploma or equivalent post-secondary education.
  • Valid Ontario Class "G" Driver's License and a current copy of a Driver's abstract for review and verification.
Experience and Knowledge:
  • Experience working with homelessness, mental health, and related issues.
  • Thorough knowledge and understanding of anti-racism, anti-oppression, and equity principles.
  • Good knowledge of community resources.
Skills and Capabilities:
  • Strong oral and written communication skills.
  • Ability to work independently and as part of a team.
  • Good interpersonal and time management skills.
