Personalized Patient Care Specialist

We are seeking a skilled and compassionate Personalized Patient Care Specialist to join our team. As a Home Health Nurse, you will provide high-quality care to patients in their homes, promoting recovery and enhancing their quality of life.
About the Role- You will assess patients' health status, administer treatments, and support families in a comfortable and familiar environment.
- Your role will involve conducting comprehensive assessments, developing individualized care plans, and monitoring patients' progress.
- Patient Assessments: Conduct thorough assessments of patients' health status, including physical exams, medical history reviews, and home environment evaluations.
- Care Planning: Develop and implement individualized care plans in collaboration with healthcare teams, tailoring interventions to meet each patient's unique needs.
- Treatment Administration: Administer medications, wound care, and other treatments as prescribed, ensuring adherence to safety and infection control protocols.
- Progress Monitoring: Monitor patients' progress and responses to treatment, making adjustments to care plans as necessary and communicating findings to healthcare providers.
- Patient Education: Educate patients and their families about managing medical conditions, medications, and necessary lifestyle changes, providing resources and support.
- Record Keeping: Maintain accurate and up-to-date patient records, documenting assessments, interventions, and patient outcomes.
- Collaboration: Coordinate care with other healthcare professionals, including physical therapists, social workers, and physicians, to ensure comprehensive support.
- Bachelor's degree in Nursing (BSN) preferred; an Associate's degree in Nursing (ADN) may be accepted with relevant experience.
- Active nursing license in British Columbia.
- Previous experience in home health nursing or a related field is preferred.
- Strong clinical assessment and critical thinking skills to manage diverse patient needs in a home setting.
- Excellent communication and interpersonal skills to build rapport with patients and families.
- Ability to work independently and manage time effectively in a home health environment.
The estimated salary for this position is $85,000 - $110,000 per year, depending on experience.
