Job Title: Social Worker I, Residential Services

About the Role

We are seeking a skilled Social Worker to join our team at Fraser Health. As a Social Worker I, Residential Services, you will play a vital role in delivering comprehensive Social Work services to clients/residents and their families within our assigned area.

Key Responsibilities

  • Conduct psycho-social assessments of clients/residents and families to identify their emotional, social, and physical needs.
  • Provide clinical counseling services to clients/residents and families to address adjustment to chronic illness, disability, relocation, bereavement, coping strategies, and caregiver support.
  • Consult and educate clients/residents, families, and interdisciplinary team members on high-risk factors, adaptation to illness and disability, loss, grief, and bereavement, transition stress, protection of vulnerable adults, community living, and support services.
  • Coordinate the admissions process and provide admission orientation to clients/residents and families.
  • Contribute to the client/resident care plan by assessing and advocating for their emotional, social, and physical needs.
  • Facilitate interdisciplinary team conferences and organize meetings with community service providers, clients/residents, and family members.
  • Provide community resource information and facilitate referrals to services in the community.
  • Maintain ongoing knowledge of community services available and contribute to research activities.

Qualifications

  • Bachelor's Degree in Social Work from an accredited School of Social Work.
  • One year's recent, related experience or an equivalent combination of education, training, and experience.
  • Current full registration with the BC College of Social Workers.
