We are seeking a skilled Social Worker I to join our Residential Services team at Fraser Health. As a key member of our interdisciplinary team, you will provide comprehensive Social Work services to clients and their families, addressing social-emotional issues, bereavement, and transition stress related to chronic illness and disability.
Key Responsibilities- Conduct psycho-social assessments of clients and families to identify their emotional, social, and physical needs.
- Provide clinical counseling services to clients and families, focusing on adjustment to chronic illness, disability, relocation, bereavement, coping strategies, and caregiver support.
- Consult and educate clients, families, and interdisciplinary team members on high-risk factors, adaptation to illness and disability, loss, grief, and bereavement, transition stress, protection of vulnerable adults, and community living and support services.
- Coordinate the admissions process, collaborating with the interdisciplinary staff and providing admission orientation to clients and families.
- Contribute to client care plans by assessing and advocating for the emotional, social, and physical needs of clients and families.
- Facilitate interdisciplinary team conferences and organize meetings with community service providers, clients, and family members as needed.
- Provide community resource information and facilitate referrals to services in the community, maintaining an ongoing knowledge of available resources.
- Contribute to research activities and participate in program planning and development.
- Maintain ongoing education and development in the Social Work profession, participating in professional development activities to advance practice.
- Bachelor's Degree in Social Work from an accredited School of Social Work.
- One year's recent, related experience or an equivalent combination of education, training, and experience.
- Current full registration with the BC College of Social Workers.
- Ability to relate to seniors and understand family systems and dynamics.
- Knowledge of residential care models, dementia, and emotional and mental health issues related to chronic illness and disability.
- Counselling and group facilitation skills, with the ability to develop, coordinate, and deliver in-services, presentations, psycho-educational, and support groups.
- Strong collaborative skills in working with community health and social agency personnel.
- Ability to work collaboratively with others, work in a multicultural environment, and understand the frameworks of other disciplines.
- Ability to communicate effectively, both verbally and in writing, and organize and set workload priorities in a complex environment.
