Job Title: Social Worker I, Residential Services

Fraser Health is seeking a skilled Social Worker I to join our team in Residential Services. As a key member of our interdisciplinary team, you will provide comprehensive Social Work services to clients and their families, addressing social-emotional issues, bereavement, transition, and adjustments due to chronic illness and disability.

Key Responsibilities:
  • Conduct psycho-social assessments of clients and families to identify their needs and develop care plans.
  • Provide clinical counseling services to clients and families, addressing adjustment to chronic illness, disability, relocation, bereavement, coping strategies, and caregiver support.
  • Consult and educate clients, families, and interdisciplinary team members on high-risk factors, adaptation to illness and disability, loss, grief, and bereavement, transition stress, protection of vulnerable adults, community living, and support services.
  • Coordinate the admissions process, providing orientation to clients and families, and contributing to client care plans by assessing and advocating for their emotional, social, and physical needs.
  • Facilitate interdisciplinary team conferences and meetings, including community service providers, clients, and family members, to address client or program needs.
  • Provide community resource information and facilitate referrals to services, maintaining an ongoing knowledge of community services available.
  • Contribute to research activities and participate in program planning and development, maintaining ongoing education and development in the Social Work profession.
Qualifications:
  • Bachelor's Degree in Social Work from an accredited School of Social Work.
  • One year's recent, related experience or an equivalent combination of education, training, and experience.
  • Current full registration with the BC College of Social Workers.
